About The Position

The Nurse Practitioner (NP) provides holistic, compassionate, and patient-centered care to adult and geriatric Members within skilled nursing and long-term care settings. As a full-time Nurse Practitioner, you will collaborate with physicians, RNs, case managers, and interdisciplinary care teams to diagnose, manage, and prevent acute and chronic conditions while promoting wellness and quality of life. This role includes accountability for Plan Medical Management, comprehensive health risk assessments, chronic disease management, and care coordination. The NP practices within the scope of state regulations, clinical protocols, and collaborative practice agreements, with a strong focus on post-acute care, provider-based care, and value-based clinical performance.

Requirements

  • 2 years of experience as a Nurse Practitioner in adult, geriatric, or family practice settings preferred.
  • Strong ability to perform comprehensive adult/geriatric assessments and manage chronic disease
  • Skilled in identifying risk factors, unmet needs, and gaps in care for senior populations
  • Proficient in medication reconciliation and managing clinically complex patients
  • Graduate of an accredited NP program (Adult/Gerontology Primary Care preferred)
  • Active RN and NP licensure in the state of practice; board certified (AANP or ANCC)
  • Current CPR certification
  • Highly organized, detail-oriented, and committed to ongoing learning and professional development

Responsibilities

  • Perform comprehensive health risk assessments for assigned Members
  • Collaborate with physicians, case managers, social workers, caregivers, and facility staff to coordinate care
  • Identify quality improvement opportunities and develop individualized care plans
  • Review and interpret medical, surgical, and social histories
  • Perform appropriate screening procedures and interpret diagnostic data
  • Recommend and implement treatment plans, including prescribing medications, ordering tests, and providing wellness counseling
  • Reconcile medications and ensure safe, appropriate use
  • Refer Members to PCPs, case management, or disease management programs as needed
  • Maintain compliance with HIPAA, confidentiality standards, and PHI regulations
  • Identify unmet needs and initiate appropriate referrals
  • Diagnose conditions and create comprehensive, evidence-based plans of care
  • Achieve clinical performance targets and quality benchmarks
  • Build positive rapport and engagement with Members and families
  • Communicate and collaborate with facility leadership to support Member care goals
  • Participate in committees related to clinical guidelines, quality improvement, and compliance
  • Maintain team schedules to ensure continuity of care
  • Travel independently to assigned facilities as required
  • Serve as a clinical leader within the facility, supporting provider-based care and post-acute care workflows
