Psych Nurse Practitioner (PMHNP) - Assisted Living Communities
About The Position
AllCare.ai delivers fully coordinated, in-facility care built around one shared care plan—Primary Care, Psychiatry, Podiatry, Diagnostics, Wound Care, and CCM—supported by a 24/7 medical concierge. Our AI-enabled tools streamline charting and compliance so clinicians can focus on person-centered care. We’re hiring PMHNP-BC clinicians to provide on-site and telehealth behavioral health services for residents in Assisted Living/RCFE communities across Stockton. Immediate need: ~300 eligible residents are ready to be seen across our Stockton assisted living communities, providing consistent, predictable volume.
Requirements
- PMHNP-BC
- active CA NP & RN licenses
- active furnishing, NPI, DEA, BLS
- EMR proficiency
Nice To Haves
- Senior living/ALF/RCFE experience preferred
Responsibilities
- Psych evals & follow-ups
- med initiation/titration/monitoring
- Coordinate with PCPs, facilities, families/POAs, and our concierge team
- Same-day documentation in EMR; regulatory/CMS compliance
- Staff education (behavior plans, safety, de-escalation basics)
