Job Highlights: A Qualified Professional (10A NCAC 27G .0104) must have one of the following: Bachelor degree (non Human Services) with 4 years Full time OR 8 years Part time applicable experience required Bachelor degree (Human Services) with 2 years Full time OR 4 years Part time applicable experience required Master degree (Human Services) with 1 year Full time OR 2 years Part time applicable experience required This Opportunity: This position assists and supports clubhouse members to increase their functioning so that they can be successful in the environments of their choice with the least amount of ongoing professional intervention. This position shares in the responsibility of operating Club Horizon’s work ordered day prevocational units, social/recreational programs and employment programs.

Requirements

  • Minimum of a Bachelor Degree (dependent upon experience) (Required)
  • Drivers License (Valid) - USA
  • Qualified Professional (QP) - Monarch-DSM
  • Experience working with adults with a mental health diagnosis and/or substance use disorder | 1-4 years (Dependent Upon Education - see Job Description) | Required

Responsibilities

  • Demonstrate good knowledge and skills of psychosocial rehabilitation as evidenced by interactions with clubhouse members and assist with their individualized goals of skill development, educational, and pre-vocational goals as identified in their person centered plans.
  • Work side-by-side with members in clubhouse units as assigned (clerical, culinary, etc.), maintaining bottom line responsibility for the work of the unit and maintain supplies for unit(s) assigned.
  • Assist the person in learning personal care skills such as healthcare, medication self-management, grooming and other life decisions affecting their illness. Assist the individual in gaining access to educational activities which include assisting the person in securing needed education services such as adult basic education and special interest courses.
  • Demonstrate good communication with clubhouse members and encourage members to participate in unit of choice.
  • Provide pre-vocational activities which focus on the development of positive work habits and participation in activities that would increase the individual’s self-worth, purpose and confidence such as member engagement, wellness programs, and community activities.
  • Assist with Transitional Employment Placement (TE) component of the clubhouse.
  • Educate TE participants about effects of working on disability benefits and assist with social security and DSS as needed.
  • Assist with training and providing support for members on job placements and document appropriately.
  • Ensure TE placement obligations are fulfilled during member absence(s). Maintain clubhouse related documentation.
  • Maintain confidentiality of clubhouse members according to Monarch policy. Maintain safety at the clubhouse as evidenced by using equipment properly, using safety precautions when needed, awareness of potential safety hazards, maintaining good safety record.
  • Demonstrate ability to work as a team member as evidenced by assisting with general clubhouse duties as needed such as: transportation, recreation, employment, and other clubhouse units.
  • Attend and actively participate in meetings and training as required.
  • Maintain certification in all agency, state, federal, and Clubhouse International training requirements.
  • Demonstrate knowledge of and comply with all agency policies and procedures, as well as state and federal statutes and regulations related to specific program areas.
  • Other duties include PCP development and updates, service note documentation, facilitation of treatment team meetings, input of authorizations, and maintaining authorizations as needed.
  • Complete all other relevant responsibilities as assigned by the supervisor.
