About The Position

Advise a caseload of assigned clients throughout all aspects of the psychiatric recovery model of treatment by providing 1 on 1 support and facilitating workshops aimed at assisting clients in overcoming their mental health barriers to successful goal acquisition Imparts agency philosophy of “Equal Opportunity, Independence and Realization of Individual Potential” to children, families, program staff and community resources. Maintains confidentiality of information relating to individuals and their families in accordance with agency policy. Guide a caseload of 16-18 clients through the rehabilitation process by assisting each client in establishing and achieving his/her personalized life role goals. Responsible for timely caseload documentation including concurrent development of clients individualized recovery plans (IRP), on-going progress notes, IRP reviews, and on-going assessment in accordance with NYSOMH regulations. Facilitates 10-12 skill-based workshops assisting clients to develop skills needed to achieve their goals. Conducts offsite visits as needed. Participates in on-going development and implementation (teaching) of program curriculum based on clients’ barriers to goal attainment. Coordinates outside referrals for clients as needed. Communicate with all members of the client’s treatment team to deliver a comprehensive coordination of care. Responsible for the orientation and intake of new clients including comprehensive client assessments. Participates in conferences, workshops, department meetings and in-service training as needed or recommended to enhance job skills and personal growth and development. Adheres to agency’s policies and procedures a well as department guidelines. Travel is primarily local during the business day but employee will be expected to work at any of our partner agencies. Some out-of-the-area and overnight travel may be expected.

Requirements

  • Bachelor’s degree in the human services field and at least 3 years of experience in public or community mental health settings serving low income and culturally diverse populations or Master’s degree in the human services field and at least 2 years of experience in public or community mental health settings serving low income and culturally diverse populations.
  • Demonstrated knowledge of a person-centered planning and ability to work as part of a fast paced multi-disciplinary team.

Nice To Haves

  • Master’s degree in the human services field (preferred)
  • Licensure in New York State as a Limited Permit MHC, LMHC or LMSW.
  • Valid NYS Driver’s License.
