About The Position

We're looking for a qualified Individual like you to join our Texas Health family. This is a grant funded position and subject to the award and continuation of the contract. It offers an exciting opportunity to work on an award-winning, collaborative community health initiative aimed at improving the health and well-being of the community. The work is connected to system changes to improve access to fresh food for all residents.

Requirements

• H.S. Diploma or Equivalent Required
• Successful completion of a Community Health Worker formal training program such as from a community college or other education institution Preferred
• 6 Months Customer service experience either face-to-face or by phone Required

Responsibilities

• Participate in program development and planning.
• Develop and effectively manage program timelines to ensure timely completion of program deliverables.
• Monitor program activities and reporting to ensure accuracy in outcome measures.
