About The Position

Netsmart exists to improve and change lives by bringing all aspects of healthcare together. We connect behavioral health, home health, hospice, rehabilitation, senior living, and social services so care teams can deliver complete, person‑centered care. The Project Manager plays a pivotal role in that mission. You will lead complex, cross‑functional projects that translate strategy into outcomes, serving as the steady guide who aligns stakeholders, clarifies goals, and builds the rhythms that make progress inevitable. Your work will remove barriers for clinicians and care teams, elevate the experience for the individuals and families we serve, and advance the promise of our CareFabric vision. This is a role for a builder and a believer—someone who blends rigor with heart. You will use automation and AI‑driven tools to streamline operations and surface insights while keeping people at the center. You will champion change with empathy, communicate with purpose, and measure what matters so teams can see the impact of their work. Success looks like projects delivered with integrity and momentum, organizations made more agile and resilient, and communities better served because of the systems you help bring to life. If you’re ready to lead with mission, collaborate across disciplines, and turn complex challenges into meaningful, measurable progress, we’d love to meet you. Apply today and help us ensure EveryDayMatters for those who deliver care—and those who depend on it.
