About The Position

Together, we’re enhancing processes for each other and optimizing outcomes for patients. The Comprehensive Health Program provides a wide range of services in English and Spanish. CHP’s care and care coordination teams oversee multi-campus efforts for HIV Testing and Prevention. Our mission is to address multiple overlapping barriers to HIV prevention and treatment by applying a multidisciplinary, patient-centered approach and delivering same-day walk-in care with robust wraparound services. The Project Leader for Wellness Initiatives will guide Wellness programs for people living with HIV, including a special focus on 50+ Wellness Programs. This role involves overseeing 50+ Project Work Plans and includes close collaboration with grant funders, the Project PI and 50+ Program staff and consumers, Quality Improvement team members and as well as collaboration with the NYP Grants Office. The Project Leader supports Program Leads, Care Coordinators and clients to plan, execute and evaluate all grant activities, working closely with the data team to generate quality of care analyses. Additionally, the Project Leader is responsible for quality control of program processes, and data collection, contributes to enrollment screenings and care planning and leads Wellness groups as well as Plus Delta sessions. The Project Leader will also support implementation of other CHP programs, providing training and leadership to team members who are implementing Wellness programming or evaluating programs. This role will ensure each project is completed within the budget and schedule parameters set by NewYork Presbyterian. It will require maintaining effective communication with department heads, staff, consultant, and outside agencies to ensure smooth operations. It will serve as a Subject Matter Expert for the team in introducing process improvements to enhance output and service while eliminating workflow inefficiencies across all Wellness Programs at CHP.
