About The Position

This position is responsible for directing the implementation and ongoing execution of the Social Care Network (SCN) in accordance with New York State Department of Health (DOH) requirements under the 1115 Waiver. The Project Director ensures that SCN deliverables are completed on time, within scope, and in compliance with state guidance and contractual expectations. The Project Director serves as the primary operational lead for the SCN and is responsible for translating executive strategy into actionable workplans, coordinating cross-functional implementation efforts, and maintaining accountability across SCN teams and contracted partners. This role provides direct supervision of SCN staff that support network development, engagement with and success of contracted entities, training, and performance to support readiness, compliance, and continuous improvement.

Requirements

  • Bachelor’s degree (Master’s preferred) in a related field, such as project management, business, public health, population health, or equivalent experience required.
  • 5-7 years of project management or related experience required
  • Experience within the New York State health care system (population health, behavioral health, Federally Qualified Health Centers, or similar) required
  • Excellent organizational skills, detail oriented to support follow through across a variety of projects and processes at once
  • Ability to independently manage multiple priorities effectively with minimal supervision
  • Proficient with Asana, HubSpot (or other CRM tool) and Microsoft Office Suite, including Microsoft Teams
  • Flexible, adaptable style that takes in stride last minute changes and project pivots
  • Able to apply established project management methodologies, tools, and best practices across the full project lifecycle.

Responsibilities

  • Provide day-to-day operational leadership for the SCN, ensuring alignment between executive direction and frontline implementation activities.
  • Define and manage SCN scope, objectives, and implementation priorities in alignment with DOH guidance, SCN Program Manual requirements, and 1115 Waiver standards.
  • Develop and oversee implementation plans, including milestones, timelines, and resource needs, in collaboration with executive leadership.
  • Monitor SCN performance and outcomes to identify trends, gaps, and opportunities for improvement.
  • Support strategic planning for post‑1115 Waiver sustainability, including operational readiness, partner capacity development, and network continuity.
  • Coordinate activities across departments, partner organizations, and geographies to support consistent execution within diverse organizational and community contexts.
  • Forecast and manage program resources efficiently in collaboration with executive leadership.
  • Provide direct supervision, coaching, and performance management for Provider Liaison Specialists (PLSs), ensuring consistent partner engagement, documentation, and accountability.
  • Supervise training lead to align onboarding, technical assistance, learning communities and partner needs.
  • Oversee network development activities through coordination and supervision of Director of Network Development
  • Coordinate with performance lead to align on performance monitoring with network compliance and partner needs.
  • Partner with technology and data team to align operational requirements with system tools ensuring compliance with requirements from OHIP, NYeC, and NYS DOH.
  • Collaborate with data and analytics team to define and operationalize performance reporting requirements to track SCN partner compliance, service delivery, and outcome performance
  • Establish and maintain structured feedback loops across SCN areas of focus
  • Monitors SCN contracted partner progress through onboarding, implementation and scaling phases.
  • Serve as the primary operational liaison with New York State DOH, leading biweekly check-in calls and ensuring accurate communication of project status and follow-up on open items.
  • Responsible for ensuring timely submission of all state reporting requirements
  • Support SCN Governing Body activities, including preparation of materials, documentation of decisions and action items, and tracking of key deliverables.
  • Perform other related duties as assigned.
