Requirements

  • Associate's Degree Preferred or Bachelor's Degree Preferred.
  • 2+ years of related experience, preferably in healthcare, public health, social work or nonprofit agency.
  • Personal and/or professional experience working with or parenting a child with special healthcare needs.

  • Experience with collaborative efforts at the statewide or community level, and understanding of related public policy and system efforts.
  • Strong interpersonal communication abilities, including effective public speaking experience and excellent writing skills.
  • Demonstrated success cultivating and sustaining partnerships and relationships among diverse organizations or groups.
  • An understanding of the factors impacting racial and ethnic disparities in health.
  • Knowledge of quality improvement tools and methodologies.
  • Knowledge of some of the regional and state resources for CYSHCN 0-21.

Responsibilities

  • Coordinates work around accomplishments of grant objectives and deliverables.
  • Provides trainings, information and referral services, and service coordination to families of children with special health care needs in the area of medical home.
  • Acts as a regional consultant to area health care practices and systems interested in aligning with best-practices in service and care coordination.
  • Supports health system and community initiatives focused on implementation of medical home quality improvement.
  • Promotes, arranges, and leads trainings and presentations on the topics of medical home and youth health transition for families, community, and health system partners.
  • Provides information, referral and follow up services to families of CYSHCN birth to 21 according to best practice standards.
  • Champions the importance of medical home to community partners including local public health department and community agencies.
  • Participates in Case Review meetings to ensure best-practices regarding delivery of service coordination and information, referral and follow-up.
  • Maintains a working knowledge of community agencies, systems, and regulations critical to the health and well-being of children with special health care needs.
  • Provides consultation to providers and professionals working with children and families in the area of government, community, and other system supports.
  • Leads the development and organization of Regional Center resources in the area of health benefits, waiver programs, and special education.
  • Participates in outreach strategies targeted at engaging families, health systems, and community partners.
  • Recruits and supports potential grantees across an eight county region interested in medical home quality improvement grants.
