This position involves performing social services functions, guiding patients in accessing therapeutic treatment skills to achieve optimal emotional health in an outpatient setting. Your responsibilities will include offering basic consultation, delivering specialized clinical social work services, crisis intervention, and conducting short-term and long-term therapy. We are seeking a dedicated individual who can provide information and referrals as needed, contributing to the overall well-being of our patients and their families. The Program Therapist is responsible for:

Requirements

  • Master's Degree from an accredited college or university, in Social Work or a clinical related mental health field preferred.
  • Licensed to practice in the State of Illinois as a mental health professional is required.
  • A minimum of two (2) years’ experience in a psychiatric health care facility, with direct experience in family and group therapies, crisis intervention, and treatment skills; and have strong clinical assessment skills preferred.
  • May be required to work flexible hours and overtime.

Responsibilities

  • Participating in team treatment meetings to represent the therapeutic needs and goals of assigned patients.
  • Performing psychosocial assessments on assigned geriatric, adult, and adolescent patients.
  • Completes indicated treatment plans using clinical information provided through the multidisciplinary assessment process.
  • Meets with patients to discuss treatment plan goals, define discharge criteria and plans, complete family sessions, and assess level of patient risk.
  • Provide daily group therapy to geriatric, adult, and adolescent populations as scheduled.
  • Serves as a liaison between team members and involved social service agencies and ensures a clinically appropriate aftercare plan is in place.
