Program Support Assistant - Case Management

Requirements
- 1 year of job-related experience in a healthcare setting
Nice To Haves
- Associate's Degree
- Bachelor's Degree
- Nursing Assistant Certification (CNA), Health Unit Coordinator (HUC), Medical Terminology upon hire
- Prior experience in discharge planning, billing, utilization review, financial counseling or registration
- Proficiency in medical terminology
Responsibilities
- Participate as an active member of the Case Management Team (CMT).
- Under the guidance of the nurse case manager and social worker, assist in facilitating appropriate patient placement.
- Utilize hospital and community resources to support continuity of patient care.
- Serve as a Utilization Management liaison.
- Collaborate effectively with Medical Staff, CMT members, the Business Office, Medical Records, and other hospital departments.
- Perform other duties as assigned.
