The Program Supervisor for Beechwood NeuroRehab is responsible for ensuring adequate staffing ratios in all Beechwood residences. Handles issues that arise during the assigned shift and collaboratively works with other departments to ensure staff has the information to ensure proper support and supervision to assigned residents. Communicates with Program Managers and Residential Director regarding areas of needed improvement in the residences as it relates to coverage and staff support.

Requirements

  • Minimum age: 21.
  • A demonstrated ability to appropriately acknowledge and respond to the multiple needs of the population served is required.
  • Must be able to lift 50 pounds.
  • Bachelor’s Degree in a Human Services field and at least 1-year experience working with individuals with a disability
  • An Associate’s Degree (or 60 credit hours) with 3-year experience with individuals with a disability.

Nice To Haves

  • Brain injury experience preferred.

Responsibilities

  • Supervises and directs all employees assigned to the 7-3 pm, 3-11 pm shift in conjunction with the appropriate PCHA AD, or RD.
  • Ensures adequate employee coverage to meet minimum staffing requirements in all residences and program areas during the 7-3 and 3-11p shifts.
  • Responsible for responding to and coordinating resolution to unusual and/or emergency situations on their assigned shift.
  • Notifies Campus Administrator/Executive Officer of all unusual incidents, when assistance is necessary for investigation of Unusual Incidents, when guidance is needed for emergency situations, or when consultation is required regarding Woods Policies and Procedures.
  • Implements emergency plans and documents activities and outcomes on designated Forms.
  • Completes the Beechwood 24-hour report to include campus coverage, reassignments, recruitment efforts, and other programmatic issues.
  • Ensures compliance with licensure and accreditation standards, including DHS 2600 and CARF Brain Injury Residential.
  • Conducts observations of all residences and communicates all reports of any individual, employee, and/or facility concern to the appropriate authority and documents issues in a timely fashion.
  • Ensures visibility in all Beechwood residences during assigned shifts and is responsible for developing positive relationships with staff and residents so that resolution of issues is maximized.
  • Coaches and mentors employees to improve job performance. Address performance issues via disciplinary actions in consultation with PCHA if warranted.
  • Responsible for addressing individual health and safety concerns which may include employee suspensions, Reporting incidents to outside agencies, and cooperating with investigations.
  • Works closely with Security, Nursing Staff, and other non-residential employees to coordinate effective communication while helping to facilitate solutions to problems/incidents relating to individuals/employees.
  • Intervenes and assists, as necessary, in individual program activities and behavioral interventions. This may include transporting individuals, administering medications, assisting individuals in ADL’s and acting as LST when needed
  • Completes daily census in CareLogic for all Beechwood residents and maintains effective communication with the PCHA’s and billing around any concerns
  • Ensures all individual/employee-related incidents receive immediate attention/follow-up. Issues discipline, as needed, in conjunction with the Campus Administrator, Human Resources Department and the appropriate Manager or Director.
  • Must become Medication Administration Certified and maintain certification over time.
  • Eventually, become a Medication Administration Trainer.
  • Must Be Smith Driver Trained.
  • Assists in conducting routine fire drills for assigned residences and documents observations on designated forms and forwards them to the appropriate Manager.
  • Performs other duties as assigned.
