The position requires the candidate to float to different skilled nursing facilities within their assigned region, working 1.0 FTE (80-hours per two-week pay period) with 8-hour day shifts and no weekends.

Requirements

  • Bachelor’s Degree in Healthcare.
  • 2+ years clinical experience in skilled nursing facility.
  • Registered Nurse licensure preferred-Active in the State of MN.
  • Clinical license in area of expertise preferred-Active in the State of MN.

Responsibilities

  • Serves as the direct liaison between Allina Health and assigned facilities to support the implementation and growth of the Preferred Provider Network.
  • Responsible for implementing strategic programs and performance improvement initiatives across assigned sites.
  • Coordinates patient placement to support hospital throughput and readmission initiatives.
  • Implements current and new programs and services.
  • Manages relationships with internal and external partners to build trust and drive engagement in strategic and operational initiatives.
  • Assists with implementation of identified programs and services at assigned sites.
  • Collaborates with site leadership and stakeholders to ensure successful adoption of identified programs.
  • Implements performance improvement initiatives.
  • Supports the implementation of multiple performance improvement initiatives.
  • Analyzes data to identify performance improvement opportunities and guide strategic design solutions.
  • Prepares and presents actionable deliverables to internal and external stakeholders.
  • Partners with external stakeholders to implement targeted action plans and monitor progress.
  • Ensures continuity and system integrity of services post-discharge from SNF through coordinated stakeholder engagement.
  • Supports Allina Hospital throughput and readmission initiatives.
  • Supports placement of patients through escalation process.
  • Supports rollout of initiatives at sites that support readmission improvement goals.
  • Proactively identifies sites not meeting census or clinical goals through site observations, relationship building and available data.
  • Other duties as assigned.
