About The Position

The Margaret M. Shea RN Adult Day Health Program (formerly known as the Mattapan Adult Day Health Program) offers families peace of mind and a support system to help them with decisions involving the care of their loved ones. Our program has provided daycare services to Boston area adults and seniors for over 28 years. For many frail elderly, adult day care allows them to live alone, or with family, postponing and often preventing nursing home placement. Our adult day program also serves adults (age 18 and over) who may have disabilities that prevent them from being completely independent. This environment offers structure to the isolated, confused, or depressed in a home-like setting. The program provides respite to the primary caregiver to be employed outside the home while their family member attends the day program. The Margaret M. Shea RN Adult Day Health Program is a program of services provided in an ambulatory, home-like setting for adults who do not require 24-hour institutional care, but because of physical and/or mental impairment, are not completely able to live independently. The Registered Nurse provides direct care to participants based on the individual’s demonstration of competency in basic nursing and nursing related skills and techniques. Coordinate patient care through the development and use of patient care and practice standards. Ensures that desired patient outcomes are usually achieved and that variances from established parameters are evaluated and addressed as necessary. Performs a wide variety of high-level activities for adult patients with chronic and terminal illness, which may include assessment, history and physical, recommendations about aggressive symptom management, developing plans to address physical and psychosocial needs, review critical path progress and update team as indicated. Requirements

• Degree in Nursing required. BSN preferred.
• Licensed to practice as a Registered Nurse in the State of Massachusetts.
• Basic Cardiac Life Support certification required.
• At least two years of recent experience in the direct care of adults or chronically disabled persons.
• Excellent communication and interpersonal skills
• Effective English writing skills to report and documentation participant’s status and observations and the care or service furnished.
• Knowledgeable of basic infection control procedures and nursing related skills such as obtaining vital signs and safety procedures.
• An RN or LPN must be onsite whenever clients are present; this may require adjustments to scheduled hours on an as needed basis.

Responsibilities

• Provision and supervision of all nursing services
• Supervision of staff
• Coordinating the completion of, and ensuring the accuracy of, each participant’s initial and ongoing assessments; completed MDS documents in a timely manner.
• Coordinating the development, &/or oversight of the ongoing review, and revision of each participant’s plan of care
• Ensuring that nursing notes are written by the staff RN’s and LPN’s at least one time per month or more often as necessary to reflect each participant’s health care status
• Responding appropriately to any acute changes in a participant’s health or functioning
• Providing appropriate health‑care education for each participant and legally authorized representative
• Assisting as necessary in the delivery of other ADH Program services; including and not limited to leading Therapeutic Activity programs
• Ensuring ongoing education, training, and professional development of all ADH staff that is relevant to care of ADH participants
• Supervising the LPNS in the obtaining, reviewing, and implementing primary care provider orders
• Coordinating the participant’s care with the primary care provider and, if applicable, community service providers
• Supervising medication management services
• Developing medication policies and procedures, in accordance with 105 CMR 158.039
• The individual must demonstrate competency in the following skill areas:
• Communication and interpersonal skill
• Observation, reporting, and documentation of participant status and the care or service furnished
• Basic infection control procedures
• Basic delegated nursing and nursing related skills including: Basic elements of body functioning and changes in body function that must be reported to the aide’s supervisor; Obtaining and recording vital signs; Personal care skills including appropriate and safe participant personal hygiene and grooming techniques; Assisting participants at various levels of functioning with activities of daily living; Basic restorative nursing; and Safety and emergency procedures, including the Heimlich Maneuver.
• Maintenance of a clean, safe, and healthy environment
• Recognizing, responding to, and reporting emergencies and knowledge of emergency procedures
• Recognizing the physical, emotional, and developmental needs of participants and working in a manner that respects participants, their privacy, and their property
• Prevention of, and reporting, participant abuse, neglect, mistreatment and misappropriation
• Caring for participants enrolled at the Program and participants with Alzheimer’s disease and related disorders
• Participate in multi-disciplinary staff meetings; leads meetings in the absence of Director and/ or as assigned.
• Working with the Social Worker, provides clinical oversight for new client referrals. Oversees all clients annual re-authorization process
• The clinical manager RN shall provide each individual who has not been determined competent pursuant to 105 CMR 158.033(F)(1)(a) with at least eight hours of orientation training regarding the skills identified at 105 CMR 158.033(F)(1)(b)(i) – (ix) before that individual may provide any direct care to participants.
• Conforms to hospital standards of performance and conduct, including those pertaining to patient rights, so that the best possible customer service and participant care may be provided.
• Assists Program Director as necessary in the operation of the program and its policies
• Performs as necessary as the Program Director when the Program Director is offsite, or on ET leave.
• All other duties as necessary to ensure the compliance and safety within the Department of Public Health’s guidelines.
