Job Summary Responsible for the coordination of the Long Term and Community Integration Services (LTCIS). Provide supervision, training, and guidance to assigned staff. Develop and implement Individual Service Plans (ISP) and provide support to assigned participants. Ensure compliance with all State and Federal regulatory guidelines, including Medicaid, Licensure, and (Commission on Accreditation of Rehabilitation Facilities) CARF, as well as other funding sources and company policy and procedure. Hire, train, and evaluate assigned staff and perform the duties as assigned by the Division Manager or designee.
