About The Position
The Population Health Program Manager plays a critical role in managing and enhancing Umpqua Health Alliance's Population Health Program. This position is responsible for the development, implementation, and oversight of initiatives aimed at improving population health outcomes, reducing health disparities, and increasing preventive care engagement. By leveraging data-driven insights, the Population Health Program Manager ensures the effective execution of population health strategies, including quality metric improvement initiatives and member outreach efforts. The primary focus of this role is to enhance the health and well-being of members by implementing population-based interventions, facilitating preventive care efforts, and fostering cross-sector collaboration with healthcare providers and community organizations.
