About The Position

Position Summary: The Program Manager of Enhanced Family Care will supervise the delivery of quality, safe and individualized services for people who fall under the DD/ABD waiver in an Enhanced Family Care environment. Additionally will ensure compliance with the He-M 1001 regulations.

Requirements

  • Bachelor’s degree preferred
  • A minimum of two years of direct or related experience required
  • Proficiency in Microsoft Office Suite and database systems
  • Valid driver’s license and auto insurance coverage to meet the Agency’s minimum requirements; use of personal vehicle to transport individuals is required

  • Comply with all state, agency and department policies and procedures; possess understanding of state regulations
  • Ability to work independently and as part of an inter-disciplinary team
  • Demonstrate a strong emphasis on customer service skills to our clients, staff and state representatives

  • Interact effectively with people of varied educational, socioeconomic and ethnic backgrounds, skill levels and value systems
  • Demonstrate excellent verbal and written communication skills; provide well written reports and documentation
  • Demonstrate proficiency in Microsoft Office, including Word, Excel, Outlook, and the ability to master new software
  • Demonstrate mediation skills and the ability to provide positive intervention in crisis situations
  • Ability to represent the Residential Department, the Area Agency and the state in a positive manner

Responsibilities

  • Hire, train and supervise direct support professionals associated with programs on your caseload in compliance with applicable regulations
  • Report all incidents of client abuse, neglect and/or exploitation to designated administrative staff and Adult Protective Services
  • Work with provider to identify and report behavioral and medical issues in a timely manner
  • Oversee care provided to client by subcontracted provider
  • Review paperwork for compliance of related state regulations such as Hem 507 and 1001, as well as internal policies of the Agency
  • Monitor provider residences for compliance of fire safety regulations
  • Prepare certified homes for state inspections and coordinate any plan of correction in conjunction with the Manager of Certification
  • Establish client goals and objectives based on evaluations and in conjunction with the individual and the team
  • Attend Service Agreement meetings and other meetings related to the individual and/or placement
  • Ensure dissemination of client information to other service team members
  • Institute services based on the individualized budget, residential proposal and the Annual Service Agreement of each individual
  • Work with providers and Nursing Department to address follow-up on medical and quality assurance issues
  • Ensure that Agency information is shared with providers
  • Provide on-call coverage as assigned
  • Provide crisis intervention and problem solving
  • Participate with team to interview and vet potential providers
  • Participate in all aspects of client moves, i.e. identify provider, set up placements, complete accompanying paperwork, coordinate client move, and monitor placement
  • Provide emergency support for providers when necessary
  • Ensure initial and/or yearly trainings are completed by providers
  • Ensure providers are in compliance with the terms of the annual contract
  • Teamwork, attendance in the office as needed, and face-to-face interaction are essential job functions
  • Complete other duties and responsibilities as assigned
