About The Position

The Program Manager, in coordination with the Senior Director of Housing Stabilization and Case Management staff, would implement and oversee a contract with the City of Santa Monica Human Services Department that is focused on rapidly resolving housing crises experienced by vulnerable individuals and families residing in the City of Santa Monica, in an attempt to prevent the loss of housing. By providing services including but not limited to one-time or short-term financial assistance, housing-related support services, legal assistance referrals, and individualized stability plans, the Program Manager will support the program by leading and implementing all outreach, financial assistance, and case management components. This position will also be responsible for building and maintaining good working relationships with City of Santa Monica social safety net program staff, as well as landlords/owners, legal assistance staff, fiscal administrator, and various community-based organizations.

Requirements

  • Bachelor’s degree OR High School Diploma/GED/Equivalent and minimum of two (2) years of case management experience in social services or similar field
  • One (1) year management/supervisory experience in social services or similar field
  • Able to communicate and interact with subcontractors, service providers and community partners in a culturally competent manner

Responsibilities

  • Initiate outreach efforts within the community, targeting areas and organizations where the highest risk participants may be located. Outreach efforts will involve program education, education surrounding local resources, and support with completion of the FFA online application.
  • Provide supervisorial support and oversight, and quality assurance for staff related to caseloads, scheduling, data integrity and contract goals or program changes.
  • Assess housing barriers of participants who are unstably housed and at-risk of homelessness to determine housing and service needs, by use of assessment and prioritization tools.
  • Develop housing, financial, and self-sufficiency case management plan with participants.
  • Complete a prioritization tool with each participant/household to determine vulnerability and risk level.
  • Determine eligibility and appropriate level of assistance based on client situation and ongoing monitoring of program spending and budget.
  • Communicate team needs, and update and maintain the tracking of reports as requested by supervisor to ensure project deliverables are being met.
  • Use progressive assistance lens when reviewing and monitoring financial requests for the program to ensure that requests are in alignment with scope of services as outlined by our funders and that each request appropriately meets the needs of program clients
  • Stay informed on the Scope of Required Services (SRS) as outlined by our funders.
  • Market, promote and network with providers in City of Santa Monica to improve program partnerships and coordination.
  • Collaborate with Central Fiscal Agent and City Right to Counsel program to submit both financial assistance requests and legal referrals in a timely manner to prevent further jeopardization of housing status.
  • Works in conjunction with legal service entities, primarily City Right to Counsel program, to facilitate the workflow between case management and legal service support.
  • Refer to credit counseling and other services necessary to assist program participants with critical skills related to budgeting, managing money, accessing a free personal credit report, and resolving personal credit problems.
  • Document all services identified and provided, referrals made, and participant feedback.
  • Advocate for and actively assists individuals in obtaining services (e.g. benefits, mental health, substance abuse, housing referrals, financial assistance, home based services, training, medical services, mentoring and socialization).
  • Ensure data is being entered into HMIS and tracked in an accurate, timely manner.
  • Keep data in HMIS, charts and productivity trackers up to date.
  • Monitoring and reporting on program outcomes and working in conjunction with fiscal agent to track program spending targets
