NewYork-Presbyterian has a long tradition of bringing hope, health and healing to our communities. NYP is continuously implementing new programs to meet the unique needs of our communities, and is strengthening those that exist. Our programs build upon, and tie together, the Hospital's inpatient care coordination, Ambulatory Care Network, and community outreach programs. Now, we're seeking a Program Manager to help drive this work forward. The Comprehensive Health Program provides a holistic system of care that includes primary medical care and essential support services for people living with HIV, including those who are uninsured or underinsured. As a Program Manager for the Comprehensive Health Program you would supervise Program Coordinators and Substance Use Counselors who conduct ongoing case management and service delivery support for patients living with HIV and Substance Use Disorders. A successful candidate for this role will understand the principals of Harm Reduction Care Models, and would benefit from familiarity with Health Home case management including service requirements and billing. The successful candidate will work directly with clinical and practice leadership to uphold service delivery requirements across multiple grant funded programs and ensure fidelity to the program's mission.

  • Bachelor's degree
  • Minimum of 5 years of experience, with minimum of 2 years in a supervisory capacity
  • Strong interpersonal skills
  • Ability to work with diverse community groups and multilevel stakeholders to drive projects and manage people.
  • Master of Public Health (MPH), Licensed Clinical Social Worker (LCSW), or equivalent with at least two years of Care Coordination Experience
  • Experience with program implementation, evaluation, and familiarity with grant reporting structure and systems
  • 5+ years of experience working in Harm Reduction Care Model or working with people who use drugs
  • Bilingual Spanish/English
