NYP's commitment to addressing the specific needs of its local communities shines brightly through the work of its Division of Community and Population Health. The Division is comprised of 14 primary care sites, seven school-based health centers, 13 mental health school-based programs, and over 60 specialty practices. It also oversees over 20 community programs, including the Health Home Program. The Comprehensive Health Program (CHP) is a center of excellence for HIV treatment and prevention as well as sexually transmitted infections, and HCV. To address social determinants of health and mitigate risk for poor outcomes among vulnerable populations, CHP has embraced comprehensive care coordination strategies and practices multidisciplinary, team-based care including integrated behavioral health services, intense care coordination and primary care. Patients are cared for by a team of multidisciplinary providers (care coordinators, SW, RN and primary care provider) and are offered Health Home enrolment as part of this comprehensive care. Health Home is funded by New York State Medicaid to address complex and high-cost medical conditions and to address medical, behavioral and social needs comprehensively. Care teams work collaboratively to develop patient-centered care plans that empower patients to be an active participant in his/her health care.

Requirements

  • Bachelor's Degree.
  • 2+ years supervisory experience.
  • Strong written and oral communication skills.
  • Experience in program development, implementation, and evaluation.

Nice To Haves

  • Bilingual English/Spanish.
  • Masters in Social Work/related clinical field or Master's in Public Health.
  • Experience in program management, development, implementation, and evaluation.
  • Interest in Quality Improvement strategies/planning.
  • Familiarity with care coordination and program management.
  • Comfort with data reports and tasking team members to achieve high quality outcomes.

Responsibilities

  • Oversee care coordination programming and Health Home program activities.
  • Lead the care coordination and Health Home programs along with another Program Manager.
  • Monitor all program requirements including billing, documentation, and data reports.
  • Guide implementation of the strategic vision of the program to ensure sustainability.
  • Provide supervision, coaching, guidance, and feedback to team care coordinators.
  • Interpret and apply findings from data reports to ensure high fidelity of data.
  • Participate in the implementation and enhancement of existing grant funded programming.
  • Supervise care coordinators and resolve clinical and administrative challenges.
  • Participate in the program's Care Coordination Steering committee meetings.
  • Collaborate with various NYP departments and community organizations.
  • Track program outcomes and report back to funders, senior leadership, and stakeholders.
