Requirements

  • Bachelor’s degree in Health Care Administration, Psychology, Social Work, Nursing, or related field.
  • 2 years of supervisory experience.
  • 2 years of relevant experience in program management.
  • Experience working in the field of HIV/AIDS, HCV, or other chronic illness treatment programs.
  • Demonstrated competency in program development, implementation, and evaluation of health related programs.
  • Experience working in grant funded programs.
  • Computer proficiency including electronic health record and Microsoft Office.

Nice To Haves

  • Masters’ degree in Health Care Administration, Public Health, Psychology, Social Work, Nursing or related field.
  • 2 years of experience administering mental health, substance abuse, or infectious disease programs.
  • Bilingual (English/Spanish).
  • Experience with New York State Department of Health AIDS Institute Reporting Software.

Responsibilities

  • Responsible for the supervision and delivery of specific Community Prevention and Treatment Services grant-funded programs.
  • Provide direct supervision for all staff members employed under the assigned CPTS projects.
  • Monitor and report statistical information and programmatic outcomes.
  • Attend trainings required for the management of goals and objectives for the grant.
  • Work with the CPTS Program Director and evaluation team to identify strategies for program sustainability.
  • Meet regularly with subcontractors and identify areas of collaboration with other community agencies and non-profits.
  • Monitor, review and sign off on initial assessments, re-assessments, service plans and case closures.
  • Perform periodic chart audits.
  • Work with the project team to identify and resolve barriers to achieving work plan objectives.
  • Oversee walk-in HIV/HCV counseling, testing and referrals (CTR) service.
  • Collaborate with community agencies to ensure timely linkage to services for client referrals.
  • Ensure data is entered timely and accurately in the electronic health record and in designated State and Federal data management systems.
  • Report all program data and program outcomes to the CPTS Program Director.
  • Track spending across assigned grants and monitor patient-level incentives.
  • Collaborate with the CPTS Program Director and support staff to develop strategies for continuous quality improvement.
  • Keep abreast of any AIRS or federal data collection changes and provide technical assistance to staff.
  • Gather, summarize, interpret, and prepare program & QI reports in a timely manner.
  • Represent CPTS programs in internal and external meetings and activities.
  • Monitor the usage and dissemination of information and educational resources.
  • Oversee the completion of all Diffused Evidence Based Interventions (DEBIs).
  • Maintain a working knowledge of significant Federal and State developments in the fields of HIV/AIDS and HCV.
  • Demonstrate excellence in both internal and external customer service.
  • Communicate HIPAA compliance, corporate compliance and client confidentiality.
  • Ensure compliance with local, state, and federal regulations.
  • Adhere to the National Patient Safety Goals as defined by the Joint Commission.
  • Complete other duties as assigned.
