Essential Job Duties and Responsibilities: Responsible for direct daily care needs of individual, ADL and recreational activities, and assistance in aiding individual learning and developing Supports in planning and executing daily activities of individuals with other staff, Lead Program Instructor, and Program Manager Implements individual’s ISP and Behavior support plans as written and documents progress as directed Encourages greater expression of choice to those we support by listening to individual’s preferences and desires Supports individuals by following their health regimen, keeping accurate documentation of medication, supervising self-administration of medication and observing and reporting their health changes Accompanies and transports individuals to participate in recreation and community outings Drives vehicle from individual or central loading area to social services or rehabilitation center, training location, job sites, or other destination according to assigned schedule Maintain a safe driving record and complies with all local traffic regulations Keeps accurate daily documentation of activities, medication, ISP tracking, as well as other documentation required Communicates professionally and treats with respect the people we support, families, service coordinators, and InCommunity employees Demonstrates and performs as an effective adult model for the people we serve Participate in at least 16 hours of online Relias training annually to remain knowledgeable on updated skills and techniques in the industry Maintains supervision of the individuals we support, unless it is stated in the Plan of Care Consult with all staff on progress and problems On duty for the hours assigned and implement planned schedule Other duties may be assigned QualificationsEducation, Certifications & Experience Requirements High School Diploma and six months experience in working in human services or related field.
