About The Position

Title: Program Facilitator
Location: Kentucky River Community Care/Perry County; Rebound Center
Employment type: Full-Time.
Education: Bachelor's degree or High School diploma with 7- 10 years of related experience.
Special Skills/Knowledge Required: Supervision Skills, Coordination skills, Planning skills, Knowledge of Substance use recovery/Peer recovery.
Shift/Days and Work Hours: Monday - Friday 8:00am - 4:30pm

Requirements

• Bachelor's degree or High School diploma with 7- 10 years of related experience.

Responsibilities

• Provide program guidance, coordination and supervision, i.e., monitor treatment intervention and progress, provide program consultative services to treatment team, attend and coordinate team meetings, identify team / unit problems and work to resolve, coordinate periodic intake, case review conferences / discharge planning etc.
• Provide therapy in accordance with agency expectations and program guidelines.
• Acts as liaison and network agent for division with other human service agencies and professionals within the assigned area.
• Serve on relevant committees.
• Participate with process improvement activities as directed by program needs
