About The Position

The Transitions Family Treatment/Rehabilitation (FT/R) program is a specialized, home-based, borough-wide prevention initiative utilizing the Solution-Based Casework (SBC) model. It serves families facing child welfare-related challenges due to substance abuse and/or mental health concerns, with the goal of repairing relationships and ensuring child safety so families can remain together. The Program Coordinator plays a key leadership role, managing and implementing essential program operations. This position works closely with the Program Director to ensure the program exceeds contractual commitments set by the NYC Administration for Childrens Services (ACS).

Requirements

  • Masters Degree required with one of the following licenses: NYS LMSW, NYS LMHC, NYS LCAT; LCSW preferred.
  • Minimum of four years experience in child welfare, mental health, or substance abuse settings.
  • Demonstrated cultural humility and anti-racist approach in child welfare practice.
  • Experience working with individuals affected by substance abuse and mental health issues.
  • Strong multitasking and administrative skills.
  • Excellent group facilitation and problem-solving abilities.
  • Proactive and initiative-driven.

Responsibilities

  • Serve as a program and agency culture carrier, advancing the FT/R and SBC models and promoting the agency's mission, vision, and values.
  • Communicate effectively across all levels of staff and external stakeholders.
  • Support positive staff morale in collaboration with the leadership team.
  • Ensure seamless operation of program functions.
  • Uphold high standards for performance excellence.
  • Recruit and hire staff across various levels in collaboration with program leadership.
  • Implement agency, funder, and model developer policies and procedures to exceed expectations.
  • Prepare timely monthly reports on program activities and outcomes.
  • Coordinate intake processes to maximize program utilization.
  • Supervise the Family Team Conference Facilitator to ensure timely and effective conferencing.
  • Schedule and facilitate Clinical Diagnostic Team (CDT) meetings per funder criteria to support clinical decision-making.
  • Provide clinical consultation on substance abuse and mental health concerns; participate in family sessions as needed.
  • Facilitate groups, workshops, and participate in program events when appropriate.
