Under the direct supervision of Residential Assistant Director, the Program Coordinator will ensure successful operations of the residential programs aligned with regulatory requirements federal/ state/county governing bodies. The Program Coordinator will act in accordance with the agency's mission and value statement concomitantly performing job responsibilities with the highest level of professionalism and enthusiasm.

  • Function as a QMHS or QIDP when necessary. Participate at IDTs & planning meetings
  • Provide highest quality of care to the Individuals serviced within the residential programs; Fully understand the person-centered approach and recovery-based model; Actively and positively communicate agency philosophy, administrative decisions, to all employees in a timely and professional manner
  • Ensure adequate supervision and staffing of each assigned location based on minimum staffing ratios and specific needs of the individuals.
  • Oversee and provide supportive direction to Residential Managers, Assistant Managers, and all staff through implementation of training, role-modeling, and structured oversight.
  • Mediate emergency program needs- deploying of staff, handling of crisis in an effective and efficient way; Communicate with the Assistant Director and other involved team members
  • Provide timely updates to the Assistant Director regarding critical issues
  • Coordinate linkages to treatment, care coordination, day activities, and other meaningful resources to enhance quality of life.
  • Actively participate in the admissions/ discharge process, as well as all team meetings.
  • Maintain certifications - SCIP/ CPR/AED/FA/AMAP
  • Assist management to evaluate, and/or modify progress or lack thereof and oversee implementation of revised goal/ treatment planning
  • Available on-call 24/7; Arranges schedule to meet the needs of the program
  • Partake in the interviewing, hiring, and training of team members
  • Supervise and approve employee hours including timecards, requests for time off, monitoringof overtime, and hours worked according to agency policy and procedures.
  • Oversee completion of critical billing documentation coupled with associated required documentation; address areas of concern; submit records in a meticulous and timelymanner.
  • Review documentation of individuals' progress on a random basis and ensure the documentation is complete and up to date; ensure that follow up occurs, as necessary.
  • Conduct regular audits of individual records, and site-based documents (fire evac. Plans, community meetings, communication logs, smoke detector checks, etc.) according to established policies and procedures- aligned with Regulatory requirements.
  • Three years of supervisor experience collaborating directly with persons with mental health, intellectual disabilities or other developmental disabilities AND is a human services professional possessing at least a bachelor's degree in a human services field, including but not limited to sociology, special education, and rehabilitation counseling or psychology.
  • Valid New York State Driver's License
  • Exerting up to twenty pounds of force occasionally and/or negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise move objects. Substantial movements (motions) of the wrists, hands, and/or fingers. The worker is required to have close visual acuity to perform an activity such as: preparing and analyzing data and figures; transcribing; viewing a computer terminal; extensive reading.
