The employee will coordinate services for customers assigned to his/her caseload as well as supervise select support staff. Services include, but are not limited to housing, finance, family relationships, social activities, education, employment, health, recreations, mobility, protective services, and records. Coordination of services also includes the development, implementation, and review of the Overall Service Plan (OSP). The OSP identifies the individualized support needed to achieve greater independence and participation in their community. The position has three main functions: provide support to customers in identifying and attaining life outcomes; supervise and direct support staff; and ensure supports provided by Community Options reflect the highest quality as defined by the accreditation standards from the Council on Quality and Leadership (CQL) and the ND DD Division.

  • Coordinate services for customers assigned to his/her caseload

  • Bachelor’s degree required, in Human Services, Developmental Disabilities, Social Work, Psychology, Behavioral Sciences, or a related field preferred.
  • For any Bachelor’s Degree not in the above-related fields, must have DD certification.
  • One (1) year of experience in developmental disabilities (DD) or a related field is required.
  • Access to working mobile phone (iPhone/ Android)
  • CPR and First Aid certification, or ability to obtain successfully during new hire training period
  • A valid driver’s license, acceptable driving record, reliable transportation, and proof of insurance for the vehicle subject to the statute of the licensing state and the ability to travel based on operational and clientele needs
  • Successfully pass a criminal background check, including Motor Vehicle Record (MVR), drug screening, and ND Department of Human Service (DHS) pre-employment approval
  • Completion of training as required by Community Options

  • Team player; good organizational and communication skills
