ESSENTIAL FUNCTIONS: 1. Ensure the development, implementation, and evaluation of an integrated treatment approach for youth with co-occurring psychiatric and substance disorders and quality trauma informed youth programs that provides culturally and linguistically appropriate services to the underserved students and families. Remain culturally sensitive and respectful to all staff and clients. 2. Provide personnel support to including program-specific orientation, onboarding, and general consultation to direct services staff to ensure contract compliance, and participate in the development of policies and procedures pertaining to personnel. Assist with oversight of staff productivity and service delivery. -Attend and assist in co-facilitating weekly multi-disciplinary team (MDT) meetings to review and support with group consultation on CAT cases. -Ensure timely documentation and maintenance of case files in accordance with SDYS policies & procedures and contract compliance. -Assist Program Manager in meeting contract statement of work requirements through processing data and audit reports minimum twice per month in ETO. -Assist Program Manager in meeting agency audit and tracking requirements including Monthly Program Numbers and Annual Outcome Reports through Apricot and ETO. 3. Collaborate closely with Program Manager and Clinical Coordinator as part of program leadership including coordinating staff coverage/support, general case consultation, and ensuring implementation of contract requirements. 4. Participate in appropriate agency-wide meetings, cross program collaborative meetings, staff development events, and committees as appropriate. -Participate in implementing agency's resource development plan as appropriate. 5. Establish and maintain linkage relationships with agencies, organizations, and civic and community groups by participating in community collaborative meeting and outreaches. -Identify and/or develop community-based resources to provide services to enhance program services, and to support healthy activities necessary for the well-being of service-partner(s). -Collaboration with Law Enforcement Departments to conduct outreach, gather Diversion referrals, conduct joint interventions, and provide on-site resources. -Initiate, coordinate, and attend community outreach events to promote program services, strengthen community partnerships, and increase referral sources. Coordinate outreach opportunities with School Administrators, Law Enforcement, and other Community Collaborators, Agencies, and Partners. 6. Maintain a case management caseload proportionate to other job duties. -Maintain all case documentation, paper files and electronic files on ETO in a timely, accurate and complete manner per contract and SDYS requirements. Include any contacts made with other service providers and family members. a. Evaluate and monitor youth progress per contract’s Statement of Work requirements and maintain accurate records including data entry into ETO and Excel spreadsheets. 7. Follow all legal and ethical guidelines pertaining to the treatment of youth and families as per the state and county requirements (e.g. Child Abuse Reporting, Duty to Warn, Confidentiality, Grievance Procedures, Informed Consent, and Serious Incident Reporting) and ensure staff do the same. 8. Other duties as necessary to ensure successful implementation of the program.
