About The Position
The position serves as coordinator of the Dementia Grant Program and Ambulatory Care department. The incumbent will lead implementation of work plan activities and will also be a Certified Medical Assistant qualification necessary to assist with direct patient care as part of their role. The coordinator will abide by guidelines set forth by grant management and the grantor. The incumbent is responsible for the overall collection, retention data, monitoring of performance improvement and generating reports. This position will facilitate and participate in the Dementia program initiatives and community outreach. The incumbent will act with independent in performance duties, conducting periodic reviews and analysis of program operations. Incumbent will participate in special and ongoing projects to ensure that goals, objectives are being met to achieve maximum efficient outcomes and requirements set by management. This is a grant funded position.
