About The Position

The Integrated Peer Models for Health Program (IPMH) is an established program of the Health Federation of Philadelphia, funded by the Pennsylvania Department of Human Services. Its mission is to utilize and support Community Health Workers (CHWs) in serving populations in the North Philadelphia Health Enterprise Zone (HEZ) who face multiple barriers to health and wellbeing, including behavioral health challenges, social drivers of health, and chronic disease. The Program Coordinator will collaborate with the IPMH Program Director and partners on the next phase of this grant, playing a crucial role in the program's administration and implementation. Key activities include leading work with partner organizations on the Connect4health resource and referral platform, coordinating professional development for CHW and Supervisor learning collaboratives, and managing the launch of a new CHW direct-service initiative, which involves hiring CHWs to address behavioral health needs within partner organizations under the Program Director's guidance. The ideal candidate is a reliable, self-starter with strong project coordination, organization, administration, and communication skills, who is collaborative and adept at learning new processes and technologies. This is a full-time position supported by grant funding for the next two years.

Requirements

  • Undergraduate degree with required skills and experience
  • 3+ years of experience doing project coordination or administrative work
  • Outstanding interpersonal skills, with a background in interacting with diverse people and organizations
  • Comfortable working in a fast-paced environment
  • Excellent written and verbal communication skills
  • Thorough attention to detail and organization skills are critical
  • Demonstrated ability to quickly and flexibly learn new processes and technologies
  • Comfortable using Microsoft Suite, Google (e-mail, calendar, Drive), and other platforms, as needed
  • Demonstrated ability to work independently

Nice To Haves

  • Master’s degree in Social Work, Public Health or other relevant study areas preferred
  • Experience working with or as a Community Health Worker or Peer Specialist in a clinical or community-based organization, or deep knowledge of Community Health Work and/or behavioral health strongly preferred
  • Some supervisory or ’lead’ experience preferred

Responsibilities

  • Implement project activities in collaboration with Project Director and partner organizations, including CHW hiring, and onboarding processes
  • Serve as a trusted touchpoint for project CHWs by providing regular check-ins to support the successful integration of CHWs into partner activities
  • Coordinate and participate in engaging and robust trainings, in collaboration with the Program Director, colleagues, partner organizations, learning collaborative members, and trainers
  • Oversee all learning collaborative session administration, including registration and follow up: Create and send out invitations/registration forms, evaluations, and participation certificates, as well as timely emails to participants and consultants ahead of and following learning collaborative sessions
  • Lead Connect4health trainings with Connect4health partners and community organizations
  • Increase the visibility and effective use of the findhelp/Connect4health platform across local community-based organizations focused on the North Philadelphia Health Enterprise Zone
  • Lead Connect4health platform engagement and utilization activities with primary partner organizations, including the creation and distribution of a Connect4health partner newsletter
  • Contribute to program planning, monitoring, evaluation, reporting and improvement activities
  • Represent the Health Federation of Philadelphia in internal and external meetings/calls and regional collaboratives
  • Document minutes and manage partner follow up communications
