About The Position

The Family Connects Coordinator oversees the daily intake responsibilities for the Family Connects Ohio (FCO) newborn home visiting model, as part of the Help Me Grow system of supports available to families across Ohio. Establishes and maintains strong relationships with local, regional, and state partners by participating in FCO technical assistance meetings and case conferences. Provides direct services and supervises staff that process referrals for the program, conducts follow-up calls and sends satisfaction surveys to families that received a FCO visit, and connects families to services and resources needed. Pulls data for assisting with generating reports and conducts quality assurance activities. Note: This position is not eligible for immigration sponsorship.

Requirements

  • Minimum of three years of experience working with pregnant individuals and/or families and young children.
  • Minimum of two years supervisory experience required, three or more years preferred.
  • Intermediate skill level in Microsoft Office applications (i.e., word processing, electronic spreadsheets, databases, and presentation software), email applications, and internet usage.
  • Ability to demonstrate a high attention to detail and good follow-up skills. Must be self-motivated, proactive, and solution-focused.
  • Ability to adhere to standards of confidentiality and professional boundaries.
  • Ability to manage multiple tasks and assignments.
  • Team building skills; organizational and staff development skills.
  • Strong interpersonal skills and the ability to express oneself effectively and concisely, both orally and in writing.
  • Ability to resolve moderately complex problems.
  • Ability to work independently with general direction.
  • Bachelor’s Degree in Early Childhood Education, Health and Human Services, Social Services, Nursing, or a related field.
  • Valid Ohio Driver’s License and appropriate insurance coverage.

Nice To Haves

  • Prior experience as a home visitor/service coordinator, nurse, and/or social worker preferred.
