About The Position

Caring for a spouse, parent, or loved one takes a great deal of time, effort, and hard work. SourcePoint’s caregiver support offers a variety of free resources and dynamic programs. The Caregiver Program Coordinator oversees this invaluable service to our community and is responsible for coordination, evaluation, and expansion of our comprehensive caregiver program. The program includes a broad array of activities and services which support, guide, educate and address the needs of caregivers served by SourcePoint. This position requires regular evening availability, with schedules flexing across operating hours Monday–Thursday (8:00 AM–8:00 PM) and Friday (8:00 AM–5:00 PM) to meet program needs. Occasional Saturday hours may be required on a rotating, quarterly basis. Flexibility in scheduling is essential to support participant access and program delivery. SourcePoint helps Delaware County adults 55 and better live safely at home and stay healthy as they age. We provide in-home care like Meals on Wheels, a large community center, support for caregivers, and more. Requirements

Demonstrate empathy, professionalism, and emotional intelligence when working with vulnerable populations
Maintain up-to-date knowledge of caregiving challenges, grief processes, and support strategies
Exhibit strong facilitation, communication, and active listening skills
Ability to manage sensitive conversations and maintain appropriate boundaries
Show initiative in identifying program improvements and new opportunities
Collaborate effectively with diverse client groups, cross-functional teams, community partners as well as navigate group dynamics
Maintain flexibility to adjust programming based on participant needs and feedback
Commit to ongoing professional development in areas related to caregiving, grief, and group facilitation
Excellent computer skills, including fluency with basic software products, including Microsoft Office
Ability to meet deadlines and successfully manage multiple tasks simultaneously
Ability to maintain accurate records and prepare professional correspondence and reports
Ability to promote a positive work culture within the organization; project a positive image of the organization in the community and to routinely interact with the public
Must have reliable transportation to and from work, and offsite events, etc., as assigned
Ability to think creatively and effectively navigate ambiguity and frequent change
Serve as person-in-charge per schedule
Associate degree in social sciences with 5 years’ experience or bachelor’s degree in related field required
Specialized training or coursework in grief, bereavement, or caregiver support required
2–5+ years of experience in one or more of the following areas: Facilitating support groups (grief, caregiver, or similar populations), Program development and implementation in a community or healthcare setting, Direct service with older adults, caregivers, or individuals experiencing loss, Behavioral health, gerontology, social services, or public health
Demonstrated experience in: Designing and leading educational workshops or support programs, Managing sensitive or emotionally complex situations with professionalism, Conducting needs assessments and using data to inform programming, Building partnerships with community organizations and stakeholders, Grief models, bereavement processes, caregiver stress, burnout, and support strategies as well as community-based services and resource navigation

Registered social work assistant or Licensed Social Worker (LSW) preferred

Responsibilities

Conduct ongoing research on best practices, emerging trends, and evidence-based models in caregiver support and grief services
Assess community needs through surveys, focus groups, and stakeholder engagement
Identify service gaps and recommend new program opportunities aligned with organizational goals
Stay informed on local, state, and national resources related to caregiving and bereavement
Design structured, evidence-informed programs for caregivers and individuals experiencing grief
Develop curriculum, session outlines, and supporting materials for workshops, educational presentations and support groups
Ensure programs are inclusive, trauma-informed, and culturally responsive
Establish clear program goals, measurable outcomes, and evaluation methods
Collaborate with internal teams and external partners to enhance program offerings
Facilitate caregiver and grief support groups, workshops, and educational sessions
Create a safe, respectful, and supportive environment for participants
Adapt facilitation style to meet diverse emotional, cultural, and learning needs
Manage group dynamics and respond appropriately to sensitive or crisis situations
Provide resource navigation and referrals as appropriate
Collect and analyze participant feedback and program data
Measure program effectiveness against established KPIs and outcomes
Use data insights to refine and improve program content and delivery
Prepare reports summarizing program impact and recommendations
Maintain a thorough understanding of programs and services provided and supported by SourcePoint. This includes SourcePoint programs and services, as well as those supported through our partnerships
Build and maintain relationships with volunteers, community partners, healthcare providers, and support organizations in the Delaware County community
Promote programs through presentations, events, and outreach efforts
Represent the organization in community meetings and collaborative initiatives
Increase awareness and accessibility of caregiver and grief support services
Maintain accurate records of program participation, outcomes, and evaluations
Ensure confidentiality and ethical standards are upheld at all times
Adhere to organizational policies and relevant regulations
Assume new responsibilities necessary to ensure a smooth continuity of operations with the organization as other organizational needs will emerge
