About The Position

The Program Consultant Healthcare Quality - Post Acute Stroke at the American Heart Association is responsible for implementing quality improvement programs aimed at enhancing care for patients with cardiovascular disease, particularly in South Dakota. This role involves collaboration with multiple healthcare sites to educate and motivate improvements in clinical practices, ensuring adherence to guidelines. The consultant will manage project timelines, budgets, and deliverables while working closely with various departments to achieve project goals.

Requirements

  • Bachelor's Degree or equivalent work experience
  • 2+ years of relevant experience
  • Experience in the post-acute clinical setting (Post Acute Stroke preferred)
  • Experience in healthcare or public health-related organizations, focusing on clinical, quality improvement, and program development
  • Proficiency in project management, including budget and timeline management
  • Strong verbal, written, and presentation communication skills
  • Ability to work in a team environment and interact with all levels of staff and volunteers
  • Comprehensive skills in Microsoft Office Suite (Word, Excel, PowerPoint, Outlook)
  • Ability to travel up to 15% and stay overnight in South Dakota.

Responsibilities

  • Drive implementation of quality improvement projects for healthcare providers treating cardiovascular disease.
  • Provide consultation and support through learning cohorts to improve patient outcomes.
  • Build relationships with internal and external stakeholders and volunteers.
  • Facilitate learning cohort meetings and local conferences in coordination with volunteers and staff partners.
  • Develop program implementation strategies aligned with initiative objectives.
  • Serve as the point person for ongoing resources needed by hospitals and clinics.
  • Establish sustainability plans for all components of the grant in coordination with staff and volunteers.
  • Develop clinical decision support tools and collaborate with clinicians to improve care.
  • Recruit program participants and work with clinical care teams to enhance workflows for better patient outcomes.
  • Communicate regularly with the Senior Manager to ensure tasks are completed on time.
  • Manage project documents, including budgets, timelines, and status reports.
