The Program Consultant Healthcare Quality - Post Acute Stroke at the American Heart Association is responsible for implementing quality programs aimed at improving care for patients with cardiovascular disease in South Dakota. This role involves working with multiple healthcare sites to educate and motivate improvements in treatment according to guidelines, managing project timelines and budgets, and collaborating with various departments to ensure project success. The position is home-based and is grant-funded, with current funding set to expire on June 30, 2027, but with the possibility of extension.

  • Drive implementation of quality improvement projects for healthcare providers treating cardiovascular disease.
  • Provide consultation, direction, and support through learning cohorts to improve patient outcomes.
  • Build relationships with internal and external stakeholders and volunteers.
  • Facilitate learning cohort meetings and local conferences in coordination with volunteers and staff partners.
  • Develop program implementation strategies aligned with initiative objectives.
  • Serve as the point person for ongoing resources needed by hospitals and clinics.
  • Establish sustainability plans for all components of the grant in coordination with staff and volunteers.
  • Develop clinical decision support tools and collaborate with clinicians to improve care.
  • Recruit program participants and work with clinical care teams to enhance workflows.
  • Communicate regularly with the Sr. Manager to ensure tasks are completed on time.
  • Manage project documents, including budgets, timelines, and status reports.
  • Bachelor's Degree or equivalent work experience
  • 2+ years of relevant experience
  • Experience in the post-acute clinical setting (Post Acute Stroke preferred)
  • Experience in healthcare or public health-related organizations
  • Proficiency in project management including budgets and timelines
  • Skilled in working with clinical audiences to impact change management
  • Strong verbal, written, and presentation communication skills
  • Ability to work in a team environment and interact with all levels of staff and volunteers
  • Comprehensive Microsoft Office suite skills
  • Ability to travel up to 15% and overnight to South Dakota
