About The Position

The Community Health (CH) Program Associate reports to the CH Program Manager. This is a supporting position to the operations of grant programs focused on planning, developing, improving, and delivering mental health prevention activities and treatment services to Native American youth and their families. Working collaboratively with the CH Program Manager and Coordinator, the Program Associate will be responsible for assisting in the development, coordination, and ensuring execution of required activities for community health programming. This includes assisting with implementation of grant-funded work plans with specific deliverables and requirements, adherence to federal, state, and organizational budget restrictions, and the collection/reporting of data required for evaluation and performance management. This role will have an assigned primary location but may be required to work at or travel to different locations as needed.

Requirements

  • Experience helping with community and/or grant funded programs.
  • Must be able to lift 40 lbs and be responsible for the set up and breakdown of outreach booth and equipment.
  • Must have a valid California Driver’s License with current auto insurance and reliable transportation.
  • Successfully pass a background check to be permitted to work with children.
  • Must be able to work nights and weekends as needed.
  • Experience in an FQHC setting as a member of a multi-disciplinary team preferred.
  • Knowledge of traditional, cultural and spiritual practices of the diverse AI/AN community, as well as ability to work with other racially, culturally, ethnically, and financially diverse populations.

Nice To Haves

  • 2 years of experience working in community and/or grant funded programs.
  • 2 years of experience in direct service in youth prevention.
  • Experience in an FQHC setting as a member of a multi-disciplinary team preferred.
  • Knowledge of traditional, cultural and spiritual practices of the diverse AI/AN community, as well as ability to work with other racially, culturally, ethnically, and financially diverse populations.

Responsibilities

  • Supports the establishment and implementation of age-appropriate, culturally responsive programs, groups, activities, and interventions related to mental health, trauma, grief and loss, and substance use and alcohol prevention.
  • Supports onsite staffing of SNAHC Gathering of Native Americans (GONA) and other community health events.
  • Ensures SNAHC services and programs are delivered in a culturally appropriate manner.
  • Complies with all state and federal laws and regulations, as they pertain to position including; HIPAA, sexual harassment, scope of practice, OSHA, etc.
  • Desire to work closely with youth and families.
  • Familiarity with client-driven care.
  • Ability to work both independently and as part of a team.
  • Excellent verbal and written communication skills.
  • Maintain accurate records and files.
  • Goal-oriented—succeeded in reaching clearly defined, time-bound goals.
  • Must have excellent organizational and computer skills including experience with word processing and spreadsheet skills.
  • With guidance from the Program Manager and Coordinator, helps implement grant funded prevention services and activities including evaluation activities.
  • Other duties as assigned.
