The Population Health Program Associate will work with the navigation team to support care management referrals, members' transitions and connections to care, and network performance. This role involves completing assessments by telephone with community members and providers to direct referrals to Brooklyn Health Home and Community-Based Organizations (CBOs) for enrollment in care management and behavioral health or social service programs. The associate may support and monitor a small caseload of members to ensure connection to services. The position requires collaboration with all levels of Central Services Organization (CSO) staff and direct communication with patients, Health Home members, downstream care management providers, community-based organizations, and managed care organizations. Responsibilities include completing behavioral health/social service assessments, assisting clients with complex needs in applying for eligible programs, managing assigned cases, tracking referrals, conducting chart reviews, and supporting Health Home referral requests.
