About The Position

To offer psychosocial support and counseling to patients and their families, helping them cope with emotional and physical challenges. To coordinate a range of services, including group therapy, crisis intervention, bereavement support, client advocacy, referrals, prevention programs, education, implementation, and psychosocial assessments. To provide supervision for undergraduate and graduate students, and to contribute to the development and execution of performance improvement initiatives.

Requirements

  • Master's degree in counseling or social work.
  • Licensed Professional Counselor (LPC) or Licensed Clinical Social Worker (LCSW) in the State of Mississippi
  • Knowledge of human behavior and performance; individual differences in ability, personality, and interests; psychosocial methods; and the assessment and treatment of behavioral and affective disorders.
  • Knowledge of principles, methods, and procedures for the diagnosis, treatment, and rehabilitation of physical and mental dysfunctions and for counseling and guidance.
  • Verbal and written communication skills.
  • Interpersonal skills.
  • Skill in the use of personal computers and related software applications.
  • Ability to gather, record, and analyze data.
  • Ability to assist individuals in recognizing and solving problems.

Responsibilities

  • Conducts and documents comprehensive psychosocial assessments to evaluate the social, emotional, environmental, and medical needs of patients and families.
  • Performs Level II evaluations for transplant applicants, donors, and individuals with mental illness or intellectual disabilities to determine discharge planning needs.
  • Maintains thorough documentation, prepares reports, and ensures compliance with departmental guidelines and standards.
  • Continuously reassesses patient needs, monitors interventions, and updates the plan of care based on progress or changing circumstances.
  • Ensures access to medical coverage by securing necessary documentation and supporting materials.
  • Provides or supervises delivery of care plan interventions including resource and referral assistance, crisis intervention, prevention services, bereavement support, counseling, therapy, education, and patient advocacy.
  • Collaborates with the interdisciplinary healthcare team to develop and implement care plans that actively involve the patient and/or family.
  • Coordinates with team members to ensure comprehensive, integrated care and to support positive clinical outcomes.
  • Identifies appropriate community resources, makes referrals, and facilitates access to external services as needed.
  • Participates in performance improvement initiatives including data collection, continuing education, policy development, and committee involvement.
  • Attends and contributes to staff, department, and administrative meetings as assigned.
  • Maintains up-to-date knowledge of local community agencies, health and social service programs, and acts as liaison when needed.
  • Stays current on medical/legal issues such as advance directives, child/elder abuse, and confidentiality requirements.
  • Supports patients in navigating financial assistance options, including applications for state benefits, charity care, and other aid programs.
