About The Position

Home Helpers Home Care is currently looking for a PRN RN to complete initial client assessments and re- evaluations of client needs. Ability to initiate care plans and necessary revisions.

Responsibilities

• Performs supervisory visits of home health aides.
• Provides services in accordance with the plan of care.
• Makes the initial evaluation visit and regularly reevaluates the client’s needs.
• Initiates the plan of care and necessary revisions.
• Provides documentation of client assessments, plan of care, supervisory visits, caregiver and client education and training.
• Informs personnel of changes in the condition and needs of the client.
• Understands and adheres to established policies and procedures
• Observes signs and symptoms and reports reactions to non-medical treatments, including changes in the patient's physical or emotional condition.
• Teaches and counsels the patient and family/significant others regarding their service needs and other related problems of the patient at home.
• Maintains accurate and complete records of observations, non-medical treatments and care of patient.
• Participates in medical record audit as assigned.
