AuthoraCare Collective is currently seeking a Hospice PRN RN for our After-hours department (Home Care/Long Term Care) supporting our Greensboro service area. The role involves assessing and identifying patient/caregiver problems, establishing nursing patient/caregiver care goals, and identifying and implementing interventions for all agency programs. The RN will document outcomes of care and work with physicians to establish and implement a medical plan of care for the patient. Effective communication with other members of the interdisciplinary group, patients, caregivers, physicians, pharmacists, and community service organizations is essential. The RN will also establish and maintain a therapeutic relationship with patients and caregivers, providing emotional support and teaching.
