About The Position

visiting angels is seeking a PRN registered nurse to complete initial and ongoing in-home assessments for clients receiving non-medical personal care and companion services. The RN develops and updates individualized plans of care and provides clinical oversight to ensure services are safe, appropriate, and compliant with state requirements for personal care. Key responsibilities Conduct comprehensive in-home assessments for new clients, including health history, functional status, psycho-social needs, and home safety. Develop and sign an individualized Plan of Care for personal care and homemaker/companion services, consistent with the client’s goals and Maryland regulations. Perform follow-up visits and reassessments (e.g., every 90 days or with significant change in condition) and update the Plan of Care accordingly. Provide clinical oversight of care delivered by non-medical caregivers and document supervisory notes as required. Communicate significant changes in client condition to the agency and, when appropriate, to family members and outside providers. Maintain accurate, timely, and complete electronic documentation for all assessments, care plans, and supervisory visits. Participate in quality assurance activities, including chart reviews and occasional staff education related to client care needs.

Requirements

  • Current, unrestricted RN license in the State of Maryland.
  • Minimum 2 years of nursing experience; home health, home care, geriatric, or community-based experience strongly preferred.
  • Demonstrated ability to perform independent home visits, assessments, and care planning.
  • Strong communication and documentation skills; comfortable collaborating with non-medical caregivers and families.
  • Reliable transportation, valid driver’s license, and ability to travel to client homes within the service area.
