About The Position

As a Home Health Physical Therapist Assistant, you'll play a vital role in empowering patients to achieve their highest level of function and independence—all within the comfort and familiarity of their own homes. Using your clinical skills and passionate care, you'll help individuals regain mobility, restore confidence, and improve their overall quality of life. Working under the supervision of a licensed Physical Therapist, you will assist in delivering both direct and indirect physical therapy interventions in accordance with the patient's individualized treatment plan and state practice guidelines. Your contributions will support recovery, reduce the risk of hospital readmissions, and foster long-term wellness. As a valued member of our interdisciplinary home health team, you will report directly to the Home Health Director. Requirements

• Graduate of an accredited program for Physical Therapy Assistants.
• Ability to read and communicate effectively in English.
• High degree of empathetic listening, tact, and kindness toward others.
• Ability to communicate well verbally.
• Strong computer knowledge.
• Ability to perform technical skills associated with procedures listed in this job description.
• Ability to work with independence and with minimal direction.

Nice To Haves

• Prior experience in the provision of physical therapy services preferred.
• Experience in acute, long term care and/or outpatient therapy.
