About The Position

The Outpatient RN CAP 2 is an entry-level position within the Clinical Advancement Program (CAP) for registered nurses. This role is responsible for providing and coordinating clinically competent care for patients, including assessment, planning, intervention, and evaluation. The RN will work as part of the End Stage Kidney Disease (ESKD) healthcare team, participating in decision-making, teaching, leadership functions, and quality improvement activities to enhance patient care outcomes.

Requirements

• Graduate of an accredited School of Nursing.
• Current appropriate state licensure as a Registered Nurse.
• Current or successful completion of CPR BLS Certification.
• Minimum of 2 years of Nephrology Nursing experience.
• Minimum of 3 months experience in chronic/acute hemodialysis as a RN.

Nice To Haves

• Experience in leadership or mentoring roles.
• Familiarity with hemodialysis treatment protocols.

Responsibilities

• Provide and coordinate clinically competent care for assigned patients.
• Perform ongoing collection and analysis of patient data during hemodialysis treatment.
• Document patient data and treatment plans in the medical record.
• Assess and document patient/family learning needs and provide education.
• Direct and collaborate with patient care staff for safe delivery of hemodialysis therapy.
• Administer medications as prescribed and document effectiveness.
• Initiate emergency response measures as needed.
• Serve as a resource and mentor for new and existing employees.
• Ensure correct laboratory collection and processing procedures are followed.
• Identify expected outcomes and update nursing assessments and care plans.
• Participate in education and quality improvement projects at the facility level.
