As a Nurse Practitioner, you serve as a trusted clinical leader and guide—supporting the hospice physician, collaborating with attending providers, and helping oversee the care of patients and families. You bring clarity to complex situations, lead with compassion, and provide expert symptom management that honors each person’s goals and dignity. Your Role in the Mission As a Nurse Practitioner at Lifted Hospice, you serve as a clinical leader within the interdisciplinary team—ensuring that every patient’s care reflects their goals, honors their dignity, and aligns with hospice standards. You partner closely with attending physicians and hospice physicians to support symptom management, guide eligibility decisions, and provide clinical oversight throughout the hospice journey. Your face-to-face visits, assessments, and expert communication help ensure appropriate, timely care. You also strengthen the team by offering consultation and education—helping nurses, social workers, chaplains, and others deliver unified, high-quality care. Through your leadership and presence, you help families feel supported, teams feel equipped, and patients feel seen and cared for at every stage. In this role, you’ll travel to see patients wherever they call home—whether that’s a private residence, assisted living, memory care, or skilled nursing facility. You’ll meet patients and families in person to complete face-to-face visits, assess clinical needs, and support care decisions with clarity and compassion. You’ll collaborate closely with your local interdisciplinary team—fellow nurses, social workers, chaplains, physicians, and our medical director—to align care plans and ensure every patient receives thoughtful, timely, and coordinated care. While your visits may vary in frequency, your presence plays a key role in the continuity, quality, and clinical integrity of our care model. This position requires local travel, flexibility in scheduling, and the ability to adapt to different care environments while maintaining professionalism, presence, and clinical excellence.

Requirements

  • Graduate of a Nurse Practitioner-focused Master’s or Doctoral nursing program from an accredited institution.
  • Active licensure as a Nurse Practitioner in the state of practice, in accordance with the Board of Nursing and applicable Nurse Licensure Compact rules.
  • Minimum of 2 years of experience as an Advanced Practice Nurse; experience in hospice, home health, or a clinical care setting is preferred.
  • Clinical assessment and decision-making skills consistent with current standards of advanced nursing practice.
  • Strong understanding of symptom management, end-of-life care, and pharmacology.
  • Excellent interpersonal and communication skills, with the ability to build trust across clinical teams and patient families.
  • Must be able to read, write, and communicate clearly in English.
  • Current driver’s license, reliable transportation, and valid auto liability insurance.

Responsibilities

  • Evaluate patient needs – Collaborate with the interdisciplinary team to assess patient status and provide clinical guidance that supports comfort, safety, and quality of life.
  • Guide plan of care development – Work closely with the team to develop and update individualized plans of care that align with patient goals and hospice standards.
  • Provide patient and family education – Help patients and families understand disease progression, symptom expectations, and how to maintain comfort at home or in their care setting.
  • Conduct and document visits – Perform and document patient and family visits in accordance with the plan of care and evolving clinical needs.
  • Collaborate with physicians – Communicate and consult with attending physicians and the hospice medical director regarding patient updates, care decisions, and clinical changes.
  • Complete face-to-face encounters – Conduct and document timely face-to-face visits prior to the third benefit period and each subsequent period, as assigned, and communicate findings to the hospice physician.
  • Strengthen the care team – Provide clinical consultation, guidance, and education to interdisciplinary team members to support unified, high-quality care.
  • Monitor and report patient changes – Identify and communicate changes in patient condition, needs, or response to care to appropriate team members and providers.
  • Maintain clinical excellence – Stay current with best practices in symptom management, comfort care, and hospice regulations through ongoing professional development.
  • Ensure safety and compliance – Follow infection control procedures, safety protocols, and Lifted Hospice policies at all times.
  • Protect confidentiality – Comply with all policies and legal standards regarding protected health information.
  • Support after-hours care – Participate in on-call rotation and respond to urgent needs as assigned to ensure continuity of care.
  • Serve as a Lifted leader – Take on additional responsibilities as assigned to support organizational needs and uphold Lifted’s mission and clinical standards.
