About The Position

The role of the Comprehensive Medication Manager (CMM) is to improve overall medication management through direct patient and provider engagement. The CMM will work to improve patient health outcomes through the increasing of patient medication adherence and creditable resolution of drug therapy problems with providers. The CMM will flex to support both the STARS: medication adherence (Stars: ADH) and medication risk management (MRM) departments as needed.

Requirements

  • LPN or RN License
  • Active, unrestricted LPN or RN license
  • Secure high-speed internet access (Ethernet connection preferred)
  • HIPAA secure work space with no background noise
  • Must be able to work Monday through Friday between hours of 8am to 5pm CST.
  • Excellent written, oral, and listening communication skills necessary to effectively interact with all levels of internal and external healthcare participants.
  • Must be able to convey information accurately while managing difficult situations and always representing AdhereHealth in a favorable manner.
  • Strong professional phone etiquette and skills.
  • Strong interpersonal and persuasion skills.
  • Ability to multi-task in a fast paced environment where changes in procedures are common.
  • Experience in interpreting policies and procedures and communicating complex topics effectively.
  • Must possess a comprehensive knowledge of medical and pharmaceutical terminology.
  • Working knowledge of web-based applications.
  • Ability to analyze, research, and problem-solve to obtain resolutions.
  • Demonstrates ability to think critically and make decisions within scope of defined role(s).
  • Proficient in Microsoft products (Word, Excel, Outlook, PPP) and ability to learn role specific software and systems.

  • Prior MCM experience preferred, but not necessary

Responsibilities

  • Support MRM program by contacting provider’s offices to follow up/send written communication of identified Drug Therapy Problems (DTPs).
  • Establish rapport by establishing professional contact with new provider’s offices explaining MRM program and associated benefits to patient health laying the groundwork for successful continued outreach.
  • Support Stars Adherence (ADH) program by contacting target populations identified as non-adherent to identify barriers preventing medication compliance and provide suggestions to overcome identified barriers.
  • Will utilize motivational interviewing techniques to engage member participation in medication adherence.
  • Calls may necessitate outreach to the member pharmacy, provider, or healthcare plan as needed to improve adherence.
  • Must maintain a quiet HIPAA secure environment (NO BACKGROUND NOISE) with direct access to secure high speed internet.
  • Must maintain expected quality measures defined in either program.
  • Establish positive partnerships across business groups both inside and outside of the organization
  • Apply sound independent judgement to develop, modify, and adapt procedures to effectively meet demands in atypical situations.
  • Participate in process improvements, including process definition, measurement analysis, and implementation of controls.
  • Participate in proactive team efforts to achieve departmental and company goals
  • Provide leadership to others through example and sharing of knowledge/skill
