As the Licensed Practical Nurse in Home Health is responsible for providing quality patient care and performing technical skilled care in the patient's home. Primary Responsibilities: Provides services within the scope of practice as defined by the state laws governing the practice of nursing, in accordance with the plan of care, and in coordination with other members of the health care team. May not alter the plan of care Instructs the clients' family in the care of the client and maintenance of a healthy environment for the client Observes, records, and reports to the supervising nurse and/or physician the patient's response to treatment and changes in the client's condition daily You'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in.

Requirements

  • Current and unrestricted LPN licensure in state of practice
  • Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation
  • CPR Certification
  • Ability to work flexible hours as required to meet identified patient needs
  • Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client
  • LA: Current LPN Licensure in LA without restrictions.
  • 1+ years of experience working as an LPN
