About The Position

At LifeStyle Options, we offer LPNs the opportunity to work with flexibility and variety. Our LPNs can choose to work PRN shifts in skilled medical facilities when they are short-staffed or provide private duty care one-on-one with clients in their homes. This role allows LPNs to keep their skills sharp while helping to reduce burnout. The position involves performing simple nursing procedures in clients' homes under the guidance of an RN Supervisor, specifically implementing nursing care based on plans developed by Registered Nurses (RNs).

Requirements

  • Graduate from an accredited school of practical nursing.
  • Licensed as a vocational/practical nurse in the state of practice (new grads welcome).
  • Home care or home health experience preferred.
  • CPR Certification (classes available for renewal).
  • Ability to provide nursing care in the home environment.
  • Ability to complete necessary documentation appropriately, legibly, and timely.
  • Interpersonal, verbal, and written communication skills necessary to complete job responsibilities.

Responsibilities

  • Perform selected nursing procedures within the scope of LVN/LPN practice including tracheostomy care, colostomy care, and g-tube care.
  • Provide client and family teaching according to the plan outlined by the RN.
  • Monitor client's vital signs and make specific measurements and observations.
  • Monitor and record intake and output, reporting significant changes immediately.
  • Observe, assess, and record client's physical and mental condition, reporting any changes.
  • Administer medications as instructed and as ordered by the physician and RN.
  • Work effectively with patients, families, and healthcare team members to coordinate patient care.
  • Maintain a high degree of confidentiality due to access to sensitive information.
  • Maintain regular, predictable, consistent attendance and be flexible to meet the needs of the agency.
  • Follow all HIPAA regulations and requirements.
  • Abide by all regulations, policies, procedures, and standards.
  • Perform other duties as assigned.
