About The Position

Empyrean Hospice hires Social Workers to implement medical social work services for individuals and the patient/family/caregiver receiving our hospice care. The Social Worker will represent Empyrean Hospice with the utmost professionalism and compassion, assessing the psychosocial status of patients and families/caregivers related to the patient's terminal illness and environment. They will conduct social evaluations, participate in the development of individualized care plans, counsel patients and families, and maintain clinical records. The role requires a commitment to ethical business and clinical practices.

Requirements

  • Professionalism, compassion, empathy, and a desire to help others
  • Self-directing with the ability to work with little direct supervision
  • Graduate of a bachelor’s program in social work accredited by the Council on Social Work Education or a master’s program in social work (MSW)
  • Must be licensed as required by Georgia law for hospice social workers
  • Minimum of one year experience in health care, hospice experience preferred
  • Knowledge and commitment to hospice philosophy
  • Understanding of grief response and experience with counseling individuals and/or in groups regarding loss
  • Demonstrate good verbal and written communication, and organization skills
  • Must be a licensed driver with an insured automobile in good working order
  • Willingness to maintain comprehensive working knowledge regarding information systems and applicable software programs

Responsibilities

  • Represent Empyrean Hospice with professionalism and compassion
  • Assess the psychosocial status of patients and families/caregivers related to the patient's terminal illness and environment
  • Conduct social evaluations including family dynamics, caregiver abilities, and communication patterns
  • Participate in the development of the individualized plan of care as part of the interdisciplinary group
  • Educate the interdisciplinary group on special needs related to the culture of the patient and family
  • Counsel patients and family/caregivers regarding stress and coping difficulties
  • Provide crisis intervention when necessary
  • Educate patients and families on advanced directives
  • Provide information and referral services regarding practical and environmental needs
  • Assist family and patient in planning for funeral arrangements and decision responsibilities
  • Maintain clinical records on all patients
  • Ensure standards of ethical business and clinical practice are prioritized
