About The Position

Empyrean Hospice hires Social Workers to implement medical social work services for individuals and the patient/family/caregiver receiving our hospice care. The Social Worker will represent Empyrean Hospice with the utmost professionalism and compassion, assessing the psychosocial status of patients and families/caregivers related to the patient's terminal illness and environment. They will communicate findings to the registered nurse and other members of the interdisciplinary group, conduct social evaluations, and participate in the development of the individualized plan of care. The role also involves counseling patients and families, educating them on advanced directives, and providing information and referral services regarding practical and environmental needs. The Social Worker will maintain clinical records and ensure standards of ethical business and clinical practice are prioritized.

Requirements

  • Professionalism, compassion, empathy, and a desire to help others
  • Self-directing with the ability to work with little direct supervision
  • Graduate of a bachelor’s program in social work accredited by the Council on Social Work Education or a master’s program in social work (MSW)
  • Licensed social worker in South Carolina
  • Minimum of one year experience in health care, hospice experience preferred
  • Knowledge and commitment to hospice philosophy
  • Understanding of grief response and experience with counseling individuals and/or in groups regarding loss
  • Good verbal and written communication, and organization skills
  • Must be a licensed driver with an insured automobile in good working order
  • Willingness to maintain knowledge regarding information systems and applicable software programs

Responsibilities

  • Represent Empyrean Hospice with professionalism and compassion
  • Assess the psychosocial status of patients and families/caregivers
  • Conduct social evaluations including family dynamics and caregiver abilities
  • Participate in the development of the individualized plan of care
  • Educate interdisciplinary group on special needs related to culture
  • Counsel patients and family/caregivers regarding stress and coping difficulties
  • Provide crisis intervention when necessary
  • Educate patients and families on advanced directives
  • Provide information and referral services for practical and environmental needs
  • Assist in planning for funeral arrangements and decision responsibilities
  • Maintain clinical records on all patients
  • Ensure ethical business and clinical practice standards are prioritized
