About The Position

The PRN Hospice Nurse Practitioner at Methodist Hospice & Family Care is responsible for providing care to hospice patients, assessing their needs, and certifying terminal illness for Medicare/Medicaid hospice services. This role involves planning and evaluating clinical management, collaborating with the interdisciplinary team, and ensuring that patients receive appropriate medical care and support during their hospice journey.

Requirements

• Master's Degree in Nursing required.
• Minimum three to five years of nursing experience required.
• Advanced Registered Nurse Practitioner (ARNP) licensure per state guidelines is required.
• Hospice certification preferred.

Responsibilities

• Provides care to hospice patients requiring assessment for terminal illness certification.
• Plans and evaluates the clinical management of clients.
• Evaluates treatment plans, medications, and necessary care.
• Acts as a consultant to the patient's physician and hospice team members.
• Certifies and recertifies patients' life expectancy in conjunction with attending physicians.
• Reviews clinical materials from referring physicians to document disease processes and health assessments.
• Evaluates needs for immediate nursing intervention and referrals to other health team members.
• Participates in formulating policies and procedures for managing health problems.
