About The Position

The PRN Hospice Chaplain and Bereavement Coordinator role at Interim HealthCare involves providing spiritual support and guidance to patients and families in a hospice setting. This position focuses on connecting patients and their families with necessary resources, counseling, and emotional support during challenging health issues. The chaplain will also coordinate bereavement services to assist families during and after their loved one's transition.

Requirements

• Graduate of an accredited seminary or school of theology (BD, MDiv, or equivalent theological degree).
• Minimum of 1 Clinical Pastoral Education (CPE) Unit.
• Experience working with death and dying, terminal individuals, and their families or caregivers.
• Hospice experience is preferred.
• Strong interpersonal skills and good communication abilities.
• Empathetic, compassionate, and resourceful.

Responsibilities

• Conduct assessments for hospice patients and their caregivers to identify spiritual needs.
• Ensure timely provision of appropriate spiritual services to patients and families.
• Provide direct spiritual services according to patients' belief systems and practices.
• Participate in the development of the interdisciplinary group (IDG) plan of care to address spiritual needs.
• Act as the hospice spiritual services community liaison by developing community contacts and offering hospice education.
• Provide funeral or memorial services for patients as requested.
• Coordinate bereavement services for patient families.
