About The Position

As a PRN Home Infusion Nurse at Naven Health, you will deliver personalized infusion care to patients in their homes or alternate treatment sites. This role emphasizes compassionate care, patient safety, and adherence to physician plans, all while working under the guidance of a Nurse Manager.

Requirements

• Graduate of an accredited School of Nursing.
• Active RN license in good standing.
• Minimum 2 years of clinical experience in infusion therapies, or 1 year with IV certification.
• Strong communication skills and a commitment to patient-centered care.
• Valid driver's license and reliable transportation for travel.

Nice To Haves

• Experience with PIVs, PICCs, and ports.
• Tech-savvy with the ability to navigate multiple platforms and applications.

Responsibilities

• Perform treatments and administer medications to patients in their homes.
• Educate patients and their families about their care and treatment plans.
• Assess patient needs and develop individualized care plans in collaboration with physicians.
• Engage with the clinical team, specialty pharmacies, patients, and caregivers to ensure effective communication and care transitions.
• Document care accurately and in real-time during patient visits.
• Safeguard patient confidentiality and uphold ethical standards in all interactions.
